NJHI Grantee, Camden Coalition of Healthcare Providers, Receives $2.8 Million Health Care Innovation Award

The Camden Coalition of Healthcare Providers at Cooper University Hospital was recently awarded a $2.8 million Health Care Innovations Award grant from the Center for Medical and Medicaid Services.

The Camden Coalition of Healthcare Providers is a current NJHI: 2011 Transitions in Care grantee, implementing systems and methods to manage and promote effective transitions in care for adult patients with multiple chronic health conditions and complex social circumstances. The Camden Coalition was also funded in 2006 by NJHI to develop its current case management model.

Cooper was the only New Jersey winner among 26 grants awarded from a field of 2,000 applicants.
