NJHI grantee Princeton HealthCare System's Partnerships for PIECE (Patient-Centered, Integrative Elder-Care and Empowerment) program was recently honored as a "Program of Promise" at the Hospital Charitable Service Awards, presented by Jackson Healthcare, the nation's third largest healthcare staffing company. New Jersey one of 14 participating states featured in ground-breaking study In this study published in the Journal of the American Medical Association (JAMA), Healthcare Quality Strategies, Inc. (HQSI), the Medicare designated Quality-Improvement Organization (QIO) for New Jersey, was one of 14 QIOs that systematically coordinated community-based interventions with hospitals to improve the quality of care transitions—when patients move from one care setting to another, such as from a hospital to their home—and avoid harmful and costly readmissions.…

Eric Coleman, MD, a geriatrician at the University of Colorado School of Medicine whose work focuses on helping patients transition from hospital to home, has won a $500,000 MacArthur Foundation "genius" grant. Coleman's work focuses on an area of healthcare that has been embraced nationally as part of health care reform -- reducing the number of patients released from a hospital only to be readmitted.…
